New Computer Setup Request Form

Directions: In order to ensure the successful setup of your new computer, please complete the form in its entirety. Once completed, please press the submit button below. You will receive an email notification thereafter indicating the ticket number that has been assigned to your request. If the form is not completed in its entirety, your new computer setup may be delayed. For a quantity of less than five computers, you must complete one form for each computer you wish to setup, in the name of the individual who will receive the equipment. If you have more than five new computers for setup on the single purchase order, please submit only one form for the entire purchase order, and place your name as the primary contact. Once authorized, we will contact you for further information regarding specifics of the setups.
